Job Description

The Screening Specialist will facilitate the initial screening of all Medicare and Medicaid patients in the clinical setting using the CMS developed data tool to ensure appropriate social services are made available to beneficiaries who meet the AHC referral criteria are in the intervention group. . Data collection and reporting will be provided to Centers for Medicare & Medicaid Services (CMS). This is a grant funded position renewed annually by CMS for a five (5) year period if all reporting requirements are met. The position is contingent annually on grant funding.

Duties and Responsibilities:

Facilitate the screening and referral process at clinical delivery sites

Document and track screening and referral services and collect data to report to CMS

Assist in training employees at patient access sites( clinical delivery sites) to administer the AHC screening using the CMS developed tool

Assist in the oversight of the screening and referral process across clinical delivery sites

Other duties as assigned


Must have High school diploma or GED with completion of an accredited medical assistant training program preferred

Excellent verbal, writing, and computer skills, particularly Microsoft Office, required
